Service Forms & Applications

Service forms for New River Light & Power (NRLP) customers.

Signatures are required on all forms. The forms are Adobe PDF format unless noted otherwise.

 New Service Application & Forms Residential Customers

Residential customers must fill out a New Service Application form in order to start service. Please complete and submit the form along with a copy of your photo ID. 

New Service Application & Forms Commercial Customers

Stop Service Disconnection Form

Residential and Commercial customers must complete and submit a disconnection form to stop service. The account must complete the form and submit with a copy of photo ID. The account holder is responsible for all charges until service is disconnected and the final bill has been paid. 

Read Only Forms

Landlords and property managers who wish to have service automatically transferred to their name when a tenant vacates the property must fill out a Read Only Service Agreement form. To cancel this service, a cancellation of read only service form must be completed and submitted.
